Forms need to be submitted using your Minnetonka student account. If you are new to the district and your Minnetonka student account is not active, you can wait until your account is activated to complete your forms. This should happen sometime in July.

- Transportation Liability Release
- Research Database Submissions
- Due on the first day of class. The Research Database submissions are your first professionalism grade.
